Stop sending every vendor the same questionnaire

Questionnaire fatigue is usually a scoping failure. The answer is not a shorter master questionnaire. It is a better decision about what needs to be asked.

The master questionnaire optimizes for the sender

A single exhaustive form is easy to administer internally, but it pushes the cost of poor scoping onto every vendor. Irrelevant questions produce rushed answers, unnecessary clarification, and evidence that analysts still have to sort manually.

Scope before you send

  • Service: What business capability depends on this vendor?
  • Data: Which classifications are stored, processed, or transmitted?
  • Access: Does the vendor reach production, identity, source code, networks, or facilities?
  • Impact: What is the consequence of outage, compromise, fraud, or data loss?
  • Subprocessors: Which fourth parties materially support the service?

Ask questions that can change the decision

Every question should influence risk, required evidence, a finding, or an approval condition. If an answer cannot change any of those outcomes, remove it or collect it later as reference data.

Use conditional sections to go deeper only when the intake response creates exposure. Production access should trigger identity and logging questions. Regulated data should trigger retention, encryption, deletion, and incident-notification questions.

Treat evidence as part of the answer

A yes/no response is rarely enough for a material control. Ask for the smallest evidence artifact that supports the decision, record its date, and define when it expires. The assessment then becomes reusable context instead of a form completed once and forgotten.
